Worminfectie — worm infection, chiefly ascariasis — was identified by Dr. C. E. Gomperts as a factor in the origin of xerophthalmie in Groot-Atjeh, and its combating stood first among his recommended preventive measures.
The mechanism proposed
Against Tijssen’s statement that in xerophthalmia “Ascariasis zag ik altijd. De beteekenis hiervan is onbekend” — I always saw ascariasis; its significance is unknown — Gomperts answered: “Vermoedelijk speelt deze infectie geen onbelangrijke rol bij de ontwikkeling van de xerophthalmie, aangezien door voedselonttrekking aan de darmen de vitamine-A toevoer aan het lichaam nog meer wordt beperkt. Bovendien zullen de excreten door de Ascariden afgescheiden naar alle waarschijnlijkheid de opname der vitaminen ook belemmeren.”
Against Tijssen’s parallel statement on keratomalacie that “Ascariasis speelt geen rol,” Gomperts replied that “deze bewering is volgens schrijver wel wat te apodictisch” — this assertion is in the writer’s view rather too apodictic.
Among the hospital cases
Worm infections figure among the infections recorded in the xerophthalmia patients nursed in the Militair Hospitaal te Koetaradja during 1936 and 1937, alongside bowel complaints, framboesia, malaria, lung disorders, pyodermia and kidney disorders.
As a preventive measure
The fifth point of Gomperts’s closing summary made the case explicitly: as a measure for preventing xerophthalmia, the combating of worm infection deserved great attention, “aangezien de wormen veel voedsel aan de darmen onttrekken, waardoor de reeds gebrekkige vitamine-A-toevoer nog meer wordt beperkt, terwijl misschien ook de excretie-producten der wormen schadelijk werken op de inwendige klieren, zoodat het evenwicht hormoon-vitamine hierdoor ook in het gedrang komt.”
The first of his five recommended measures was accordingly “eene krachtige bestrijding van de worminfecties door het verstrekken van wormtabletten” — a vigorous combating of the worm infections by the distribution of worm tablets. The expenditure statement for the survey records 36,650 tabletten santonini calomel distributed at a value of ƒ113.615, and 2.58 kilogrammes of Oleum Chenopodii — both anthelmintics — at ƒ33.54.
